New Patient Forms
We want your first visit with us to be stress-free in every possible way. To ensure the check-in process is as simple as possible, we've made our new patient forms available online. To expedite your appointment, simply print your forms out at home and complete them before arriving. If you have any questions about our forms, feel free to contact us.
Your First Visit
If you're visiting our dental office for the first time, we want you to have a full understanding of what you can expect from your appointment.
When you arrive, our front office team members will greet you and provide any paperwork necessary to check in. While you wait for your appointment, you can enjoy a complimentary cup of coffee while waiting in our relaxing patient reception lounge.
Once it's time for your dental appointment, the hygienist will lead you back to one of our treatment areas and make sure you're comfortably seated. Your appointment begins with X-rays and a thorough dental cleaning. We may also use an intraoral camera to capture images of your mouth. Finally, you'll meet our dentist, who will examine your mouth for signs of the most common oral health problems.
The information we collect during your visit gives us a comprehensive picture of your health needs. As a result, we end your appointment by recommending additional care, if necessary. We can also talk to you about financing, if needed.
If you would prefer to fill out your forms in our office, that's fine, too. However, we do ask that you arrive at least 15 minutes early, so you'll have plenty of time to complete them. This helps us keep your appointment, as well as others, on schedule.
Dental Insurance
We understand how important insurance is to our patients. That's why we'll gladly file a claim on your behalf, in addition to answering questions you may have regarding your benefits. To learn more about our policy on insurance, you can refer to the form above, or give us a call.
We accept most major PPO dental insurances and we will be happy to file a claim on your behalf. We verify all dental insurance benefits prior to your appointment, so that we may help you better understand your coverage. We're dedicated to helping patients maximize their dental insurance benefits.
Accepted Payments
To make paying for care as easy as possible, we accept payment with cash, check, Visa, MasterCard, American Express, Discover, and Apple Pay.
Flexible Financing
In addition to accepting a wide range of payment options, we also accept financing with CareCredit. Through this helpful third-party lender, patients receive zero-interest loans or reasonable, long-term financing that can make expensive procedures more affordable. We never want anyone to put off essential care because of what it costs. And thanks to CareCredit, patients don't have to make that difficult choice. Sleep Appliance Forms
For our sleep medicine patients, you may be eligible to use your medical insurance benefits towards your treatment. Our insurance coordinator will work with you to determine the best use of your insurance benefits. Additional paperwork may be required. If directed by our insurance coordinator, you may complete your sleep paperwork below.
